Session 1: ‘An Introduction to Frailty’
Session Overview: This session aims to discuss and debunk the term ‘Frailty’ and the importance of identifying frailty in the community. We will also discuss how to support those living with frailty and patient engagement. This is an introductory session suitable for all members of the primary care team including clinical staff of all grades, social prescribers, health and wellbeing coaches and any non-clinical staff who have a role in supporting patients and service users who are at risk of or living with frailty.
The session is facilitated by Charlotte Marley.
