Job Summary

We are seeking a compassionate and proactive Social Prescriber to join our team. As a Social Prescriber, you will support patients by connecting them to community groups, healthcare, and statutory services for both practical and emotional support. Your role is crucial in helping patients make informed health choices, build resilience, and address factors impacting their health such as social isolation, poor housing, and physical inactivity.

Key Responsibilities:

• Connect individuals to community-based activities and services that address their practical, social, and emotional needs.

• Provide personalised support to help patients manage their wellbeing, live independently, and improve health outcomes.

• Create tailored plans with patients to improve their health and wellbeing, linking them to appropriate community and statutory services.

• Manage and prioritise caseloads based on individual needs and urgency.

• Work within safeguarding procedures and build effective relationships with GP practices and local community groups.

• Promote social prescribing and its benefits across the community and local agencies.

• Use appropriate systems for data capture, case management, feedback, and reporting, adhering to data protection legislation.

Other Responsibilities:

• Work in various locations, including occasional home visits, requiring independent travel within Worcestershire.

• Provide one-to-one mentoring as needed.

• Commit to personal, professional, and organisational development.

• Occasional evening and weekend work may be required.

Qualifications and Experience:

Essential:

• Experience in a person-centred role (including unpaid work)

• Proven ability to recognise and manage risk

• Knowledge of safeguarding practices

• Excellent interpersonal and communication skills

• Ability to work independently and as part of a team

• Good time management and ability to prioritise tasks

• IT skills, including Microsoft Office

• Experience in health and social care

• Experience working with individuals with complex needs

Desirable:

• Training in motivational coaching and interviewing or equivalent experience

• Experience using EMIS clinical system

• Background in community development, adult health and social care, learning support, or public health/health improvement (including unpaid work)

Collaborative Working Relationships:

• Work effectively as part of a team, recognising the roles of colleagues and other healthcare professionals.

• Communicate effectively with patients, carers, and staff at all levels.

• Liaise with other GP practices and staff for the collective benefit of patients.

Confidentiality:

• Maintain strict confidentiality of patient, staff, and organisational information.

• Follow practice policies and procedures relating to confidentiality and data protection.

Health & Safety:

• Promote and maintain health, safety, and security in the workplace.

• Use personal security systems and follow infection control procedures.

• Report health and safety hazards immediately.

• Undertake periodic infection control training.
