The Role:

We are looking for a dedicated Care Co-ordinator to join our team. You will play a crucial role in coordinating care for residents of care homes, helping them and their families understand and manage their personal care and support plans. This position involves working closely with GP practices to provide seamless, patient-centered care.

Key Responsibilities:

• Identify patients needing care packages and coordinate their support
• Develop and maintain personalized care plans in collaboration with patients, carers, and healthcare professionals
• Assist patients in managing appointments and accessing necessary support services
• Provide information and guidance to help patients make informed decisions about their care
• Coordinate care across various health and social care services
• Maintain accurate records and ensure timely updates to care plans

Qualifications and Experience:

Essential:

• Good general standard of education
• Experience working with individuals with complex health and care needs
• Strong communication and interpersonal skills
• Ability to manage time effectively and work independently
• Knowledge of safeguarding, confidentiality, and GDPR

Desirable:

• Health and social care qualification
• Experience in a multi-disciplinary team
• Familiarity with healthcare systems like EMIS

Location:

Your base will be at Grey Gable Surgery, Inkberrow, with travel required among PCN member practices and business locations in Worcestershire. Some elements of the role may allow for home working. Flexibility in working hours, including some evenings and weekends, is required.
