About Us: Vale of Evesham PCN is dedicated to improving patient care and tackling health inequalities. We're looking for enthusiastic Care Co-ordinators to play a key role in providing support and ensuring residents receive the care they need.

What You’ll Do:

• Co-ordinate care for residents of care homes, ensuring their needs are addressed.
• Develop and manage personalised care and support plans.
• Work closely with GP practices and the Primary Care Network (PCN).
• Support patients and their families in understanding and managing their health and wellbeing.
• Collaborate with healthcare professionals to ensure seamless and integrated support.

Key Responsibilities:

• Identify and manage a caseload of patients in need of care packages.
• Engage with patients, carers, and advocates to develop holistic care plans.
• Provide guidance on managing health and accessing support services.
• Liaise with local health and care services to co-ordinate care.
• Maintain up-to-date records and ensure all care plans are followed.

Qualifications & Experience:

• Essential:
• Good general standard of education.
• Experience working with people to empower them and improve their wellbeing.
• Strong communication and interpersonal skills.
• Ability to work on own initiative and manage time effectively.
• Knowledge of healthcare systems, safeguarding, and confidentiality.
• Desirable:
• Health and social care qualification.
• Experience in a multidisciplinary team.
• Familiarity with issues related to dementia and complex health needs.
• Use of IT systems including EMIS and Microsoft Office.

Location: Merstow Green Medical Practice, Evesham, covering Evesham Town, Broadway Bredon, and Inkberrow. Some travel within Worcestershire may be required.
