About the Role

Worcester City PCN is looking for a Work and Health Employment Coach to support individuals facing health-related barriers to employment. This role offers a unique opportunity to be part of the WorkWell programme, working with local communities to provide coaching, guidance, and practical support to help people return to work or remain in employment.

As part of a wider county-wide network of employment coaches, you will manage a caseload of individuals with physical, mental, or social health challenges affecting their ability to work. You will assess their needs, develop action plans, provide one-to-one coaching, and refer them to relevant health and employment services. No clinical experience is required, and full training will be provided.

Key Responsibilities

  • Provide tailored coaching and support to individuals struggling to remain in or return to work.
  • Conduct assessments to identify employment barriers and create action plans.
  • Work collaboratively with GPs, Job Centre Plus, employers, and voluntary sector organisations.
  • Deliver one-to-one sessions and group workshops to support participants.
  • Maintain detailed case records and action plans, ensuring accurate data collection.
  • Refer participants to external services for additional health and employment support.
  • Support training for external organisations to raise awareness of WorkWell and employment challenges.
  • Travel across the PCN area to meet participants unable to attend in-person sessions.

 

Person Specification

Essential Criteria:

  • Experience in coaching, mentoring, social prescribing, or employment support.
  • Understanding of health inequalities and employment challenges.
  • Strong interpersonal and communication skills.
  • Ability to work independently and within a team.
  • Commitment to confidentiality, equality, and social inclusion.
  • Flexibility to travel between multiple locations as required.

Desirable Criteria:

  • Experience working in Primary Care or healthcare settings.
  • Qualification in Health or Employment Coaching.
  • Knowledge of the Equality Act 2010 and employment legislation.
