Primary Care Nurse - Maternity Cover - 12 Months

Maintain CPD by attending study days courses appropriate to the needs of Davenal House surgery.
Implement and evaluate individual treatment plans for chronic disease patients.
Provide chronic disease clinics, delivering patient care as necessary using evidenced based practise, referring patients to secondary or specialist care as required following discussion with NP/GP/PA.
Identify, manage and support patients at risk of developing long term conditions, preventing adverse effects to patients health.
Provide routine nursing care and guidance to patients as required in accordance with clinical based evidence eg National Institute for Health and Care Excellence.
