Clinical Pharmacist

Our surgery has 13,000 patients and the clinical team consists of 5 Partners, 4 salaried GP's, a large ANP/Paramedic/Specialist Nursing team, HCA's and Phlebotomist, and Pharmacist. The practice is located very near to the M5 and M42, excellent for commuting.

The successful applicant will support the Surgery in meeting its obligations under the PCN Network Enhanced Service and will work alongside our existing pharmacist at New Road.
The responsibilities will include conducting Structured Medication Reviews and Medicines Optimisation to tackle polypharmacy, audit and review of medicines through NHS-led programmes such as Low Priority Prescribing and CCG driven cost saving switching.
For an appropriate candidate, there will be potential scope for professional development in evolving skills in independent prescribing and through specialist clinics including asthma/COPD, family planning, diabetes and community based anticoagulant monitoring.
Core Duties:
The Clinical Pharmacist will be expected to:
· Work collaboratively within the Surgery with existing Clinical Pharmacist in dealing with day-to-day medication queries and reviews and liaising with GP’s, other practice staff and members of the primary healthcare team to manage patient care and through this support high quality, safe, cost effective, rational and evidence-based prescribing.
· Work collaboratively within the wider PCN network with existing Clinical Pharmacists in dealing with PCN and CCG driven medication programmes and supporting Care Home Teams with prescribing and deprescribing advice and Structured Medication Reviews.
· Evolve and establish flexible critical thinking in the clinical decision-making processes needed to deliver safe and effective care in modern General Practice
· Work as part of a multi-disciplinary team in a patient facing role to clinically assess and treat patients using their expert knowledge of medicines for specific disease areas.
· Provide specialist expertise in the use of medicines while helping to address both the public health and social care needs of patients in the network and help in tackling inequalities.
· Undertake audits as required by the PCN and implement appropriate measures to effectively act on the results.
· Through structured medication reviews, support patients to take their medications to get the best from them, reduce waste and promote self-care.
Other Roles and Responsibilities.
· Support further integration of general practice with the wider healthcare teams (including community and hospital pharmacy teams) to help improve patient outcomes, ensure better access to healthcare and help manage general practice workload.
· Develop relationships and work closely with other pharmacists across networks and the wider health system.
· Participate in regular supervision sessions and annual appraisal with line manager and clinical supervisor.
· Attend training and development activities as identified and participate in meetings as required.
· Work flexibly to meet the needs of patients.
· To undertake any other duties appropriate to the grade and purpose of the job as may be agreed by the post holder.
· Act on own professional judgement on a daily basis with regard to making recommendations in respect of medicines management
· To work as a clinical pharmacist within the practice and within defined local protocols that have been approved by GPs and existing pharmacist team.
· To be responsible and accountable for own actions, working independently within professional and defined organisational boundaries.
· Plan and prioritise own workload.
· Initiate and manage meetings with prescribers and other practice staff and make records as appropriate.
· Undertake any other duties that are commensurate with the post and within the post holder’s clinical competence.
